Our History

Founded in 1985, The Shoulder has been providing drug/alcohol treatment to Gulf Coast communities for 40 years! The Shoulder's first operational facility opened in 1988 on the historic Mobile Bay Causeway. Following the devastation wrought by Hurricane Katrina, The Shoulder relocated to its new home farther up the hill in Spanish Fort. In 2021, while the COVID pandemic forced many organizations to shutter, The Shoulder expanded its services and opened its Mobile facility. The Shoulder now operates two facilities with an overall capacity of 82 beds!

Assessments

The Shoulder offers assessments for $75 for individuals wishing to be considered for residential treatment. Assessments are required prior to admission into any of The Shoulder's programs.

Special Consideration Given To:

Individuals who are pregnant and have IV substance use disorders
Individuals who are pregnant and have substance use disorders

Programs at The Shoulder

Level III.5 — High Intensity Residential — Women's Facility 16 Beds | Mobile, AL

This is a comprehensive treatment program for adult women only. Clients reside at The Shoulder and participate in structured clinical activities throughout the day including: individual and group counseling, spiritual development, life skills, and community recovery groups. Treatment focuses on three pillars: spirituality, recovery orientation, and the twelve steps. The Shoulder has 16 beds in this program in Mobile.

Level III.1 — Low Intensity Residential — Both Facilities 32 Beds (Men's) / 26 Beds (Women's)

This is a comprehensive treatment program for adults only. Clients reside at The Shoulder, are able to work during the day and take part in therapeutic activities during the evenings including: individual and group counseling, spiritual development, life skills, and community recovery groups. Treatment focuses on three pillars: spirituality, recovery orientation, and the twelve steps. Family members are welcome to visit clients on Sunday afternoons after attending a one-hour family group. The Men's facility has 32 beds in this program. The Women's facility has 26 beds in this program.

Level III.01 — Transitional Residential — Men's Facility 8 Beds | Spanish Fort, AL

Upon successful completion of the Low Intensity Residential Program, it is recommended that you step down and transition into the Transitional Program. The Transitional Residential Program allows you more independence yet continues to provide much-needed structure and support. This transition helps you slowly integrate back into the community without being overwhelmed. The Shoulder has 8 beds in this program in Spanish Fort.

Alumni Program

After graduation from the residential programs, clients have the opportunity to return weekly to process life issues in a supportive group setting.

Operation VETS

The Shoulder offers services specifically designed to support veterans while in treatment. Our staff includes counselors who are veterans and are well versed in the unique challenges that our servicemen and women face. Our recovery support services includes groups for veterans, led by veterans, as well as integration into the broader recovery community.

Community Education

The Shoulder is committed to reducing the stigma associated with addiction. We offer various educational workshops and outreach events to educate the public about substance use disorders and how to access care. We have spoken to elementary schools, churches, and community events.

Outpatient Program

The Shoulder is taking appointments for our new low intensity Outpatient Program. Individuals seeking supportive counseling following residential treatment, or for other reasons, may contact us at 251-626-2199.

Yes, an assessment is required for admission to the Residential Program. Please be prepared for a UA alcohol/drug test prior to your scheduled assessment. Failure to provide a sample will result in cancellation of the assessment appointment. Assessments at The Shoulder are $75. To schedule an assessment, call 251-626-2199. If you have an assessment from another agency, a copy of that assessment must be faxed to 251-626-2388 to determine if The Shoulder will be an appropriate place for treatment.

All potential clients must have a valid picture ID. Each client will need to have a ready to work physical and have a doctor authorize any medications (prescribed and over the counter) that you will be taking while at The Shoulder. Clients must be working and be able to provide a recent paycheck stub.

The initial down payment is $550, $750 for 3.5. This covers the initial cost of entry into treatment and is applied directly to your fees so you don't fall behind while trying to obtain employment. Weekly fees are calculated on a sliding fee scale based on your income. At the time of admission, you will complete a budget worksheet with a staff member. That budget worksheet takes into consideration your income and monthly expenses. Your weekly fee will be 75% of your disposable income. Disposable income is calculated by subtracting your expenses from your income. What is left is considered your disposable income. Proof of income is to be presented at time of admission and then weekly or biweekly (depending on your pay periods). Documentation acceptable as proof of income include check stubs, bank account statement showing deposit, or written statement from employer with contact information for verification purposes.

Advanced permission is required before you are allowed to keep a vehicle on the premises. If you are allowed to have your personal vehicle, you must turn your keys in to the PA station any time you are on campus. In order to drive you must provide documentation verifying that you are eligible to do so. The following documents must be presented at the PA station before you will be allowed to drive or even keep a vehicle on The Shoulder property:

  • Valid Driver's License
  • Proof of Insurance
  • Valid registration (in your name)

If the vehicle is not registered in your name, a letter from the owner authorizing you to use the vehicle must be submitted as well. If you purchase a vehicle while in treatment, you may temporarily submit the Bill of Sale in place of the registration. However, you must obtain and submit a valid registration to the PA station within 30 days of the date of purchase. You must notify the PA desk of any changes to your transportation situation including (insurance, driver's license, vehicle, etc.)

We have found that the use of electronic devices, such as Smart Phones, Tablets, PCs, iWatches, and other wireless devices pose a considerable distraction from treatment. At the time of admission all electronic devices will be surrendered to the PA Station where they will be catalogued and secured. If employed, you may sign your phone out at 6 AM, and sign your phone back in no later than 6 PM. If you have a scheduled appointment, such as a medical appointment or a job interview, you may sign your phone out for that occasion. Should you need to check messages on your device, i.e. awaiting to hear from a prospective employer, you may check your messages at the PA Station. The Shoulder is not responsible for charging your device therefore it is your responsibility to have your device charged and turned off prior to turning it in.

III.01 Clients may have their electronic devices however they are to remain in their designated living area (upstairs) unless leaving for work, passes, or appointments. They are prohibited out on campus.

Storage space is limited. Please only bring what will fit into one dresser and small closet area.

Personal clothing: Weather appropriate clothing, nothing with offensive or derogatory messages or images

Bedding (twin size): Sheets, blankets, pillows

Other: Valid photo ID, plastic storage bin, laundry detergent, towels and washcloths, alarm clock, laundry bag, clothes hangers, medications prescribed by a doctor (No narcotic medications are allowed on campus.)

  • Alcohol, drugs (including Tianaa/ZaZa), or drug paraphernalia
  • Refillable vapes (unopened, disposable vapes are allowed)
  • DVDs or movies which promote the use of drugs or have an inappropriate theme
  • Personal TVs or other entertainment/viewing devices
  • Weapons of any kind (including knives and hunting equipment)
  • Energy drinks (i.e. Red Bull, Monster, Redline, Bang, 5-hour Energy, etc.)

Clients are able to participate in a recreational therapy group once a month. The Shoulder requires clients to obtain and maintain full time (40 hours) employment while in the program. Fees for treatment at The Shoulder are based on a sliding fee scale based on client income. No one will ever be turned away solely on their ability to pay for treatment.

High Intensity Residential Program

The High-Intensity Residential treatment program at The Shoulder is designed to provide the clients with 24-hour supportive treatment in a contained, safe, and structured environment to assist clients in initiating or continuing their journey toward recovery. Each client will have access to support services as needed such as: a physician, emergency services, and a registered nurse twenty-four (24) hours a day, seven (7) days per week. Clients in this program will receive a minimum of 25 hours of clinical services per week.

The stability afforded through predictable income allows us to spend more time on innovating our programs and services and raising awareness of The Shoulder's mission and advocacy for the communities we serve.

Practically speaking, monthly giving is also easier to fold into your budget than a one-time gift may be. It allows you to choose an amount that works for you and ensures you can help support The Shoulder all year long.

On average, we spend approximately $179,000 per month for general operating expenses, ranging from facilities maintenance costs to staffing, expenses related to providing our clients, transportation and meals, to the fees associated with postage and mailing, and much more. Including monthly sustaining donations, all donors are encouraged to help The Shoulder raise more than 62% toward our monthly expenses.

This donation amount allows us to provide scholarships to clients who are unable to cover the cost of assessment and initial fees related to treatment. Our goal is to have 50 or more Leadership Circle donors that helps to ensure the long term success of The Shoulder.

The Shoulder relies on donations to provide quality, affordable treatment to individuals with substance use disorders. We offer a scholarship program to eligible clients who present for services, meet diagnostic criteria for residential-level care, and do not have the financial resources available to pay for treatment. The Shoulder is always in need of personal care items for clients including personal hygiene kits, sheets and blankets as well as funds to purchase educational material to assist clients in their recovery.

In the process, we may share your information with internal staff needed to complete such request(s). In addition, we may share any non-identifying information for statistical or analytic purposes and all personal information can be shared within the organization.

Donor information is kept on file for IRS purposes. It is also used to analyze overall giving patterns in order to make more accurate budget projections, as well as to understand donors' interests in our mission and to update them on the organization's plans and activities. This information is shared with staff, board members, and consultants only on a confidential and need-to-know basis.

Discontinuing Contact Upon Request

It is the policy of The Shoulder to communicate with donors according to their expressed preferences whenever possible. The Shoulder shall maintain a record of all requests by persons who indicate to The Shoulder, its professional fundraiser, or other agents, that they do not wish to be contacted by or on behalf of The Shoulder.

Upon a person's (or a person's authorized representative's) request that The Shoulder of the Central Gulf Coast, Incorporated discontinue further contacts, the person's name and address will be promptly modified in The Shoulder's donor database to ensure that no further contact is made with the person. The Shoulder will also take steps to ensure that no further contact is made with the individual and that the person's name is removed from any external databases or records under The Shoulder's control.

Permanent Record: The Shoulder of the Central Gulf Coast, Incorporated will maintain an electronic record of all requests for discontinuance of contacts, effective with the adoption of this policy by The Shoulder's board of directors. Oral requests will be noted in writing by The Shoulder staff then permanently recorded in the electronic donor database. The records of persons who have made such a request will be maintained by The Shoulder to the extent necessary for legal or liability purposes.
